Screening Guidelines
Screenings for height, weight, vision, hearing, postural (spine) development and substance abuse are scheduled according to MA state regulations. Appropriate screenings occur in preschool, 1st, 4th, 7th, and 10th grades. Parents are notified if an abnormality is found.
Parents and legal guardians may request, in writing, that their child not participate in any screening.

PHYSICAL EXAMINATION REQUIREMENTS
Mansfield Public School requires Physical Examinations to be provided for entry into Preschool or Kindergarten, Grade 4, Grade 7, and Grade 10.
A Physical Examination completed within one year prior to school entry is required for any
new student entering without a Massachusetts school health record (from out of state) or from
within Massachusetts if a recent physical is not included in the transferring school health record.
All Physical Examinations must be performed by a Licensed Medical Physician, or a
Physician’s Assistant or Nurse Practitioner practicing under the provisions of existing state
law.
Parents/guardians with insufficient or no health insurance are asked to contact the
school nurse.
