Massachusetts law (M.G.L. c 71, s 57 and related amendments and regulations 105 CMR 200.000- 200.920) require physical examinations of school children within 13 months before entry into school or during the first year after entry, and at intervals of either 3 or 4 years thereafter, such as Kindergarten, 4th grade, 7th grade and 10th grade.
Mansfield Public School requires for all students during the following increments:
- One year prior or 30 days after entry to the Mansfield Public Schools
- Entering grades preschool, K, 4, 7, and 10
- Annually prior to participation in school sponsored athletics
- At the high school level, a student participating in interscholastic athletics must submit a current physical exam

New students to the Mansfield Public Schools, in compliance with regulations of the Massachusetts Department of Public Health, are required to provide a current physical (within 13 months of entrance) and up-to-date immunizations for school entry. New students need a copy of a physical exam that contains documentation of a vision screening, TB risk assessment, and immunization record.

Routine

Routine physical documentation is required in grades K,4,7,& 10. It is encouraged that a new physical exam be submitted to the school nurse every year after well visit checkups. Please review your student’s documentation and have any condition which is not current, removed, otherwise your nurse may reach out to you for information on a condition that no longer exists.
Students entering from countries other than the United States, Canada, Australia, New Zealand or Western Europe should also submit a TB Risk Assessment Form completed by your physician.

Physical Exams:
High School Sports

Students must have an up to date physical exam on file with the school nurse prior to sign up and tryouts in order to be eligible to participate in school sports.
Per the State Department of Public Health regulations and RULE 56 of the MIAA HANDBOOK, physical exams are valid for 13 months from the date of exam, NO EXCEPTIONS. A student’s eligibility will terminate once a physical has reached the 13 month limit.
STUDENTS WILL NOT BE ALLOWED TO PARTICIPATE IN SPORTS WITHOUT A VALID PHYSICAL EXAM ON FILE IN THE NURSE’S OFFICE.

Immunizations:
All students entering grade 7 must have received a Tdap booster. Students who are not in compliance will be excluded from school until documentation is submitted with proof of the vaccine. STUDENTS WITHOUT DOCUMENTATION OF THIS IMMUNIZATION AFTER POSTED DEADLINE WILL BE PROHIBITED FROM SCHOOL.

All students entering grade 11 must have received the meningococcal vaccine
(MenACWY) booster. Students who are not in compliance will be excluded from
school until documentation is submitted with proof of the vaccine. STUDENTS WITHOUT DOCUMENTATION OF THIS IMMUNIZATION AFTER POSTED DEADLINE WILL BE PROHIBITED FROM SCHOOL.
